Participation Waiver & Consent
By registering my child for ESTEEM Education camp activities, I acknowledge that participation in STEM and cosmetic chemistry activities may involve supervised handling of cosmetic-grade ingredients, scientific tools, fragrances, oils, dyes, butters, waxes, soaps, and related materials.
I understand there may be inherent risks including minor cuts, burns, spills, skin irritation, allergic reactions, or general risks associated with youth educational and recreational programming.
I voluntarily permit my child to participate in all camp activities and agree that ESTEEM Education, its instructors, employees, volunteers, and affiliates shall not be held liable except where required by law.
I confirm that I have disclosed all allergies, medical conditions, accessibility needs, and relevant medical information.
I understand that ESTEEM Education may photograph or record camp activities for educational and promotional purposes.
I authorize ESTEEM Education staff to provide emergency first aid, contact emergency services if necessary, and obtain emergency medical treatment if I cannot be reached immediately.
I acknowledge responsibility for any resulting medical expenses and confirm that the medical information provided is accurate.
